1) Old
B) Not a tech inspection issue
III) Shop sold car owner wrong lug nuts
d) Lug nuts threaded up by hand OK enough to be tightened by cordless impact.
IV) Despite what seemed like being tightened enough, the "close but wrong thread" couldn't withstand transitional forces.
Moral: know the thread pitch and don't trust someone else to know it if you don't. [All Mazdas have had the same metric thread pitch at least as far back as the 1st Gen RX-7. Shop sold the unsuspecting owner a very close SAE lug nut.]

Sorry, i saw the date it was posted to YouTube was recently so I figured it was new.

Yeah... 1/2-20 lug nuts do not work well on 12mm-1.5 studs.
