Where do I find the "minor" waver?

Samuel, seven, will be with us at the event on Saturday, where do I find the waver?

Ok, I have the "Minor Release and Waver of Liability and Indemnity Agreement".
Only one form?

You should print two copies and have them both signed. One goes to SCCA and one you keep. Also, for the date, put "all events 2012," so you can use it again this year.
